HHD 5783 Name Badge Tickets Form

Thank you for your interest in PTS' 2022 High Holy Day In-Person Worship Services!

Each household is entitled to 2 name badges (aka tickets) for the head(s) of household and unlimited name badges for those ages 5-29 (no name badges needed for younger children). Name badges will be mailed shortly after Labor Day to those whose accounts are current and who have registered by August 15. If you have not renewed your 2022-23 membership or your account is not current, please contact the office to make arrangements. Name badges will be issued only after arrangements are made.

Name badges (aka tickets) will be mailed only to those who provide proof of vaccination. As of early summer, proof of vaccination for ages 5+ will be required to attend in-person services. Skip the lines; do a vaccination pre-check. Name badges will be mailed to all those who have pre-checked. All others will pick up name badges at will-call.

Three ways to pre-check:
- Sign the Vaccination Pre-Check Register when you attend a PTS event this summer
- Show your vaccination card on Zoom in August. See sholom.org/hhd for times and links.
- Stop by the office to show your card

If you plan to bring guests, you will use this SAME form to order their name badges (aka tickets). Note that guest name badges for ages 30 and up are $250/person for the entire HHD season. If you plan to bring guests ages 30 and up and this presents a financial burden, we are able to help. Please contact office@sholom.org BEFORE completing this registration.
