We look forward to your event! Before completing this Program Request Form, please look at the PTS calendar at sholom.org/calendar to see if your preferred date/time looks available. Note that behind-the-scenes activities may impact actual availability. After you submit, you will receive an email with further instructions. You will have an opportunity to share additional information or ask questions as needed.

If your event is multi-part or complicated, please complete this form as much as you can.

If you have a change to a previously submitted event, please resubmit the form below or send an email to brochelle@sholom.org with the words, "CHANGE " in the subject line.

For special events, subject to scheduling availability, PTS may send one or more special e-blasts as follows:

0-20 attendees: no special e-blast
21-50 attendees: 1 special e-blast
51-100 attendees: up to 2 special e-blasts
>100 attendees: up to 3 special e-blasts
