Memorial plaques may be purchased by PTS members for loved ones and by non-members who are remembering those who were PTS congregants when they died.

Thank you for remembering your loved one with a permanent memorial plaque. Your plaque purchase of $700 includes the following:

• Custom designed plaque added to the wall
• A marker will be placed on your loved one’s plaque each year on their yahrzeit 
• For as long as you remain a member of PTS, your loved one’s name will be read during the Shabbat of their yahrzeit 
• Your loved one’s name will be added to the Yom Kippur Book of Remembrance for as long as there is a Book of Remembrance

Note that plaques will be added to the wall once per year in the summer. They will be sorted alphabetically by family until the wall is full. In the future, when a new wall is added, plaques will be sorted alphabetically by year. 

When you submit this, $700 will be billed to your PTS account. You will have the option to pay this amount after you click “Continue to Payment.” Or, you may “bill to account” and send a check to 1655 Sebastian Drive, Burlingame, CA 94010. 

Each memorial plaque includes the name of the deceased loved one, the English date of death and the Hebrew date of death.
